Overview of Case Management Service: Ottawa Salus offers a range of intensive case management services in English, French, and ASL (American Sign Language). Salus is a partner agency in the Mental Health Community Support Services coordinated by the Canadian Mental Health Association which operates the intake and assessment portion of the service.
What Do Case Managers Do? Based on the strengths model of case management delivery, Salus case managers provide individualized, flexible, and client directed support to individuals suffering from severe and persistent mental illness. The focus is upon client strengths, interests and abilities. The service is provided in the client’s environment and is portable, following the client when he/she changes housing.
Why? The goal of case management is to enable individuals to participate more fully in their community with an improved quality of life.
How? The case manager and client assess client strengths and needs in all life domain areas, identify goals for change, and develop a service plan. The case manager then assists the client in meeting goals by providing supportive counseling, practical assistance, coordination and linking of services, as well as advocacy.
Specialized Case Management Services:
Concurrent Disorder Services: Intensive case management in English & French to individuals with both addiction and mental health concerns.
Deaf, Deafened & Hard of Hearing Services: Intensive case management provided in ASL (American Sign Language) for individuals with mental health concerns.
Step Up Program: Long term support to Salus clients who are recovering from a severe and persistent mental health problem but have attained a higher level of stability and autonomy. As a result, Step Up clients no longer require active / intensive case management. Services focus on social, vocational, and recreational goals as well as traditional case management tasks as needed. Step Up members can continue to be involved in all Salus activities and events. In addition, the Step Up Program offers regular recreational events that are planned and organized by its members.
